• If you have health insurance, please capture a photo of both the front and back of your insurance card.

What Happens Next?

• A staff member will contact you to schedule the soonest available appointment time.
• You will have a brief video or phone appointment with a physician who will prescribe your Suboxone medications.
• Your prescriptions will be sent to your pharmacy for pickup following your video call.
